• Providing clinical information or a referral form confirming disability.
• Offering a referral form duly completed by a treating facility or practitioner if previously rejected on medical grounds.
• Undergoing a Means Test, including the applicant and their spouse.
• Not being maintained or cared for in a State-funded institution.
• Not receiving another social grant for themselves.
• Submitting a 13-digit bar-coded identity document or a smart ID card for both the applicant and their spouse. In cases where these forms of identification are unavailable, an alternative prescribed by Sassa will be accepted.
